database creation from online subscription form

Internet viewers log on to our website and register as a member. Currenty, membership information is being handled manually. We would like this information to go into an excel spreadsheet as a database. We would also like to have a 'refer a friend' where the viewer can list a friend's name and email address. There are about 10 fields that need to be recorded. Current system works on MYSQL.
