Create a dealer registration database

I am in need of a client registration database that will allow potential client to request a login and password for entry to a members room that will enable them to design electronic systems online. The database will capture information from an online registration form. *Please only apply if you have specific MySQL experience and are committed to sticking with a project once it has started.
