We need a form in which people can enter information (name, age, address, tel, email, etc). The information from this form should get a unique code attached and be stored in a DB and in a CSV or .XLS file. The information also needs to be output to a pdf file (again, with unique code) for the user to print out, sign and send off to us. (The pdf should also be sent to their email). To summarize, the info: 1. gets a unique code per subscriber 2. is stored in DB and XLS 3. is output in a pdf for the user to sign and send. We need someone who can do this perfectly, securely and by the 10th February.
