I need the Chess program

I need one form put on one web site so that six people in differenet places can add either a 1 or a 0 to each of 20 lines. This is a score sheet for a competition. Our academy conducts online chess tournaments. Have you seen a standard pairing form? one-to-one we want a form that we can place on our website next to the pairing form where the observing coaches can enter the results. We need a simple result form with board numbers and a line for the observing coaches to enter the results. That results form will be placed on the website next to the pairing form. The pairing for is generated from the paring software. Not something we can work with. No access to source code. So we need something like this ... Just the right hand results form. We have 6 coaches who enter results into the results column. The form needs to be active and live and seen by anyone the has the page loaded. I want to be able to insert the form code into the page coding. I don't recall ever seeing an online form which numerous people can make entries in real time for all others can see the entries. It seems like the process will have to include a write function then a refresh ... hummm. two could be making entries at the same time Our next tournament is Saturday. Coaches training and testing is on Friday. Will I be able to insert the code within may HTML page code. More than one place on the same page where we have more than one pairing table. I hope you email the code or I can provide an FTP site. Certainly this is not a large solution. if your solution is a wrapper table than a plain page to a ftp site would work.
