Report Writing Application

Looking for a developer to create a ASP or PHP based web application (not fussed) which allows for the generation of customer reports based on a title/tick box approach of issues which are accessible from a database (mysql or ms-sql - again not fussed). The report should output into a customized Microsoft Word report template which is available for download. The application will support RBAC through a simple username and password logon. The application will need to support two roles - managers, and authors. The application will need to support multiple users (Approx 15 initial users expanding in the future). Manager roles will support the creation of projects/jobs and also allow authoring. Only managers will be allowed to create jobs/projects. The job will need to have standard customer details added (Company name, job number, job type, customer name, address, telephone, email etc). Managers will allow for only certain users to access the job or project. Managers will also have the ability to add issues to the database, as well as edit issues. Author roles will allow users to access the application, select the job currently assigned to them, select a number of tick boxes through the method of a horizontal drop and pop menu which are available from the database. It should have the option to be saved (to return in the future and be edited) as well as save (to return in the future and be edited) and generate a report. The application once submitted should generate a Microsoft word document report using the custom template which is made for immediate download The application will be required to be extended in the future with additional features and support so code should be clean and well documented throughout.
