I am working with a medical doctor who is a specialist in a particular field. Over many years, he has developed a 17 questions questionnaire that will provide an accurate but general diagnosis of the condition in which he specialises.
Then, if people want a more specialised diagnosis, they need to complete an additional 34 question questionnaire which will give them a diagnosis of which of the six sub-type(s) of the condition they have. Based on this diagnosis a treatment plan is provided.
The intention is to make this a web based self-service system whereby the first questionnaire and general diagnosis will be free (with delivery of result both instantaneously on screen and confirming email).
Then, if they want the more specialized diagnosis and specific treatment plan, they will need to pay (probably by PayPal or credit card via [login to view URL]) and then be able to access the 34 questions at the end of which they receive delivery of result (diagnosis and relevant treatment plan) both instantaneously on screen and confirming email.
The questionnaires have been simplified in such a way that there are up to five possible answers and each answer has a different weighting which is additive. Therefore, once complete, the cumulative score will fall into one of four diagnosis bands ie, Insignificant, Mild, Significant and Very Significant.
The questions, answers, number of questions and weightings may change over time and therefore the web-based solution would ideally provide the ability for us to be able to make changes without help of a coder.
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
## Platform
Users must be able to run it in all common browsers. We have our own server at
<[login to view URL]> running Ensim and it must be able to be hosted on this.
