Web App/ Widget - repost

I'm looking for someone to build a nice looking and functional web application or widget that I can host on my website [login to view URL] and others can copy and paste into their websites. I have a working version of this application here: [login to view URL] The goal is to let people embed HTML code into their site so their site visitors can calculate pounds for cubic foot and obtain the freight class for that calculation. My site is built in PHP and I would also need a single page where people can test the app and copy the HTML code for their personal use. I have a few ideas and requirements for style and layout that I will explain once a freelancer is chosen. A good example of the page and application style is located here: [login to view URL] This app calculates cost, not what I need, but gives you an idea of the completed project. Also, my widget will not require a login in order to use. You'll get FTP access to the site where there is a folder that already contains the basic script for the freight class calculator.
