253583 Contest form and download

I need a form with the following fields: Name Email Address Mobile number The name and email are required fields. The mobile number is not required. The form should have error checking to make sure that the email address is in valid email format. Once they submit the form the data should be stored in a database and then an email should automatically be sent to the email address with a unique link that will allow them to download a secure mp3 song. You can build this on your server, we can test and then I'll need you to place it onto my server.
