115419 PHP proof reader and advisor

I am having some issues with formgen, in that it doesn't generate forms. So I've tried to make the forms myself, copying formgen's format. I'd like you to help me find my error. The form is fine design wise, but it doesn't connect to the database and doesn't save the data. There are two forms with which I need your help, each consisting of one html form and one [login to view URL] file. For someone who knows what they are doing I anticipate this will take quite a short time, but I hope it will result in a long working relationship where you will be able to advise me in the future.
