Build a Website for a database of Doctors

A developer is needed to build a web application “Doctor’s website and database” that will have a database of doctors with various data regarding them and any visitors/potential patients who visit the website can search through it using various criteria, can request appointments for certain doctors and give feedback/rate them. The application will have a control panel for the administrator where he can add/modify/remove entries, add/modify/disable rating criteria, enable/disable comment feedback for certain doctors or for all, add/modify/remove doctor’s specialties. Criterias, specialties, university titles, rating criterias will be dynamic, the administrator will be able to add new ones or disable unwanted ones. Doctors will be split into 3 groups. Each “doctor” entry will have associated following details: Name, date of birth, age, sex, education, main specialty, other specialties, level of specialization, university title, known foreign languages, Address (*5 - five locations), Google Maps Location coordinates(*5), phone number (*5 - for each location), Group, IsFeedbackAllowed, Ratings(*5 - for each location), Location type(*5 - for each location), Pictures, Other details. Doctors from one of the groups will have access to a “doctor control panel” where they can edit their own details, locations, upload pictures for their entry. Each address entry contains County, City, Street, Number, Floor, Room No., zip code. A visitor of the website will be able to search for doctors by any criteria mentioned above, including by distance from a certain location, eg. 5km from X location. Results must be shown instantly, while the visitor types in the search or chooses from the search criterias. Website’s design needs to be responsive and usable on mobile browsers as well. HTML5, CSS3, Ajax has to be used, retrieving info and data via Ajax is a key element.
