Main items i want is 1) Nicer checkboxes 2) Nicer radio buttons 3) Nice Labels 4) Fields that enables the view of other fields based on the option) should be highlighted or underlined in the interface. 5) Field that have the option of "To Confirm" selected should be of different color. 6) Summary at the end of all the fields with "To Confirm" Selected.
