Create a module for Joomla v3, only interface for booking

It has to be module for joomla 3 characteristic similar as jPanel module for joomla 3. Module must have position, one is closed (picture 1a) and second is open (picture 2). It has to be floating module. That means that when we are scrolling page down, module must be always on the left botom of page. I need only interface for booking, joomla module with php and css files. It has to be interface with 2 calendar fields like in picture 2 and 3 (the first calendar for arrival date and the other calendar for departure date). I have all codes for comunication with server (php files). I have [login to view URL] page, code that is working with booking server. So, that interface we have to integrate in joomla floating module on page. For the remaining details (php details) I will send privately. Thank you.
