Edit custom contact7 form on existing website

I have an existing website that I admin for a client. It has a simple contact form created by contact form 7 on each page of the website. Lately we have been getting tons of spam messages. (15-20 each day) I need to add a simple quiz to the form to keep spam from filing through the site. I have done this on many of my own sites but I did not design this site so it is not easy for me with my skills to accomplish this. All i need is for someone to add this simple quiz question to validate the form before submission. The only hard requirement is that it has to look like the rest of the form. I have attached a picture of the form with the edits so you can see how simple this job is. You will only have wordpress access to the website as well as FTP access if requested. No cpanel access will be granted. If you cannot do the job without cPanel access do not bid on the job... period.
