I have an event website that I need a survey form added to it. Below are the questions and options I need. 1) Do you plan on attending NYE 2021? YES NO 2) Would you attend NYE 2021 if it were at the Wyndham Garden Dallas North Hotel again? YES NO 3) If a new Hotel or Venue were identified but we would have to scale back attendence from the usual 800 to 1000 people down to 400 to 500 max, would you still attend? YES NO 4) What type of Entertainment would you like to see this year; ie. Live band, DJs, Burlesque or visual performances, etc? (make answers 100 words or less) 5) What type of theme would you like to see? (make angers 100 words or less) 6) Is on-site food at any hour at the hotel or venue very important?
