Website Form

This project doesn't necessarily need to be done with WooCommerce. I'm open to suggestions. Also, I've set a price here but really have no idea the cost so will compare bids and skills of contractor. This form needs to be created on an existing wordpress site. The math needs to work. I've included a PDF and Excel doc, they are the same thing. Once the data is submitted it needs to be emailed to an administrator in a tab or CSV format and should be kept in a database somewhere. Then the client can import it into their database. It should be clean for client to print out and/or preferably they get an email of the information. Once the client has filled out all the information they'll need be directed to a paypal link. The client changes this form every year so there will be subsequent work on it if desired.
