Wordpress and Mail Chimp Mailing List Plugin Expert Needed ASAP

Hello, We have an urgent project. We need someone who is an expert in WordPress and mailing list plugins, specifically mail chimp, to help us out immediately. What we need is to add two different mailing list sign up forms to our website. The first is very basic and just asks for their email address and then has the submit/sign up button. After they submit, they will be added to the "General Mailing List" and will get a message that says "You have been successfully added to the Sports Card [login to view URL] Mailing List!" . The second form asks for the following info: *Full Name *Email *Type of Collector? (drop down) 1 - Hobby Collector - This is for fun, not about the investment 2 - Investment Collector - Love the hobby but also look at sports cards as an investment 3 - Flipper - I love the hobby and I try to make money by buying/selling/flipping cards 4 - Dealer - I am so passionate about the hobby that it's my job * Comments on why you want early access (large text box) *Are you willing to give feedback on the matching engine? (yes/no option) *Submit (button) After the user submits, this info will get added to the "General Mailing List" as well as a separate "Beta Mailing List". They will get a message that says "Thank you for signing up for early access and to test our Matching Engine. Please check your email for instructions on how to access the matching engine." Then Mailchimp will send them a welcome email with the following message: "Thank you for signing up to test out and provide feedback on the Sports Card [login to view URL] Matching Engine. We appreciate your help and look forward to hearing how it works! You can access the matching engine at [login to view URL]" You are responsible for setting up the lists in Mail Chimp (we will provide you a user login) as well as implementing the forms onto our wordpress site. The signup forms must look good and have the correct color scheme per our current website look and feel, www.sportscardscanner.com. This is an urgent project and we are willing to hire someone ASAP and we need this completed in the next few hours. Please provide examples to demonstrate you have the skills to set this up.
