Web Page Programming Online Calculator and Contact Us Form

I need the code from my current online calculator at [[login to view URL]][1] to be added to the attach page and the new page coded for placement [[login to view URL]][2].? I own both sites and [[login to view URL]][2] is? a live version of the site currently in progress. The new page will function similar to the [login to view URL] as far as the calculation goes.? When an item is selected it will appear in the selected items at the bottom of the page.? When calculate is pressed the price per person and meal price will appear.? Also if name, email, or phone has a character in it, it sends me an email with their selected items, price and contact info. I also need a contact us page coded.
