DVD Inlay design

Project for a designer with DTP experience. Please, bid only if You have it. I have an e-box designed. Now I need a professional inlay for a real DVD case to match this e-box. Selected designer will be given the elements to use: logo, text, screenshots, award graphics. Final product should be provided in print-ready form, with all the requirements (CMYK, bleed, fonts converted to curves, etc). Format - vector based with embedded photos and graphic elements. It can be AI (not flattened), EPS or PDF (but not Photoshop PDF export). Thank You.
