Employee ID Badge Design

I am looking for someone to design an employee ID photo badge for our company. I will use this as a template and add the photo, names and IDs here. I am going to want a few mock up ideas before selecting the final design and I have attached a screenshot of the layout I have in mind as well as a PDF of our business card for color and possibly background ideas. The ID badge will only require the following information: Employee Name, Title, Employee ID, Company Name and Phone Number along with a photo. The badge size should be 3" wide X 2.5" high.
