Update HTML and CSS for web app

We have a Live Chat system under development and need to incorporate some HTML/CSS developed for the customer chat window into a functioning prototype. The prototype includes use of Angular JS, WebSync, JQuery, and custom commands from our proprietary iService forms interface. We had a designer create the PSD and HTML for the various views of the chat form (offline message, pre-chat survey. active chat window, etc.). We have incorporate the basic HTML from those designs into the prototype application and need someone to clean up the CSS and HTML to match the design mockups. Please note that since the customer chat form is rendered by the iService forms interface, you cannot just update the HTML and load it in a browser to view it. The updated form MUST be loaded into iService and viewed as an iService form generated by our CRM system. We will provide you a login and password so you can paste your updated code into a form and preview it for testing. The following are attached for your review prior to bidding. This document includes comments that should help you understand its components. Please review this document and ensure you understand how it functions. There may be some small changes required, but in general it is what the final [login to view URL] should look like. The goal for you is to make the [login to view URL] look like the chat designs in the ZIP file.
