Web HTML5/Page/DIV Design using HTML5 (webkit supported)
The app utilizes jqMobi and jqUI for its base functionality to be used on mobile devices (IOS, Android). Utilizing these libraries we have setup the base framework of page formatting. Each Tab on the NAVBAR represents its own div section.

DIV Section 1 (HOME): Will have our logo and text within the section explaining proxy (we will enter the text), and have 2 IOS Like buttons. One Blue, One Green. (2) Javascript stubs shall be left for us to populate our own JS.
DIV Section 2 (SIGNUP): This Div Section will have a form based section to allow user to enter account information for signing up. Format of the form shall look like:

Changinging the following:
Step 1: Account Info
Login Id
Email
Password
Password again
Step 2: First Name
Last Name
Birthdate (using html5 picker)
Country (using drop list)
Step 3: TOS
Check box stating user accepts Proxys Terms of Service
Button (IOS like) to present a popup displaying our Terms of Service
SUBMIT Button - Join
Instead of a normal SUBMIT using a form postback, we want a Javascript stub, so we can do our own AJAX Call to create the account.
DIV Section 3 (ACTIVATION):
A paragraph of our text will be placed here, then using the same for above for the form, we will have a form with 2 field (username and password) and a submit button, again with a Javascript function stub, where we can place our own code to resend the activation email.
DIV Section 4 (LOST):
A paragraph of our text will be placed here, then using the same for above for the form, we will have a form with 2 fields (username and email addres) and a submit button, again with a Javascript function stub, where we can place our own code to send an email with a code allowing a password reset.
DIV Section 5 (RESET):
A paragraph of our text will be placed here, then using the same for above for the form, we will have a form with 2 sections:
Section 1: (Like Step 1: Account Info) 2 fields (username and reset code)
Section 2: (Like Step 2: New Password) 2 fields (password and password again)
and a submit button, again with a Javascript function stub, where we can place our own code to change the password.

Attached is the the project used to create this site.
Each PANEL (DIV SECTION ABOVE) will be in its own file not part of the [login to view URL]
The PANELS get loaded upon viewing. So all pertaining javascript, and html in the div should be in its own file. Please refer to the div id ="navabr" section, and look at the SIGNUP to see the use of loading of the div section from a url (in the [login to view URL])
Please ignore the [login to view URL] file in this project.
Additionally, ONE (1) additional style sheet should be added (not added to an existing style sheet in the project) to provide the formatting for these 5 DIV sections.
For the HOME tab, use the attached ICON as the LOGO for now.
For sections requiring "text", just put 4-5 lines of text, anything is fine, we will replace.
Appearance, layout, and clean html/javascript expected.
Need to be able to start ASAP, not looking for extended amount of time to complete, this is actually a very easy job.
