Simple calculator with: 5 input elements and 4 outputs. The 5 input elements will be free text The free text will be used to where necessary to ‘lookup a cost of goods’ The customer input on ‘number of intubations’ and will be used to calculate VMI’s offer The customers current spend will be used to calculate the saving. If the saving is 0 or below the following message will be displayed ‘ Contact us ................’ to discuss how we can save you money and help your patients and clinicians benefit from Video Laryngoscopy. All out puts will be in $ The calculator will not provide a result until a ll the elements are complete including Name / hospital and email address / country /region The result will be automatically emailed to our email as a comparative result The results will be provided on a separate page and include the following information: • Small body of text • Current spend based on total number of intubations / annual spend • Saving based on total number of intubations x VMI’s identified ASP • Call to action including email the result VMI will need back end access to alter prices. The calculator will be hosted on a landing page.

Hi, To Summarize the requirement, 1)Customers will input on 4 free text sections to get the output of cost of goods. 2)The text fields will be auto corrected based on the backend added texts 3)At end backend the cost of goods would be assigned. 4)Customers will input the total number of intubations 5)Savings will be calculated based on total number of intubations x VMI 6)Admin can update the VMI from backend. 7)The result will be emailed to client and Admin.
