Single page test app to replace user input in word document and convert to pdf

I am currently working on an iPad project and due to time constraints, need some help with one part of my app. The project is a business e-commerce app and I need to take the user input details such as name, address and products and create a pdf contract/invoice. Freelancer task is to create a single page app with input fields for name, address, comment and three product descriptions and prices. The replace the fields in the attached word document and create a pdf. You can either edit and replace in word, generate a word document from scratch or create the first page of the document which is dynamic and create it straight in a pdf file and combine with page 2. The object is to get the user info from the app to the pdf in this EXACT format. The attached file is a dummy template. Final version will be more complicated.
