Simple application

we want to build a simple application for our referral program .Our plan to partner up with different kiosks all across the city and have them hand out business cards that offer a 10% discount for mobile device repairs .Now In order for us to efficiently execute this plan we need a application that can: 1. Read the QR code on the business card when the customer come in. This Code should hold the business name ,location and phone number . (put illfixit in the bid comment so we know you have gone through the description) 2. So once we scan the QR code we need the application to populate the appropriate fields with in the application and leave fields to put in the customers information and the repair being performed along wit the cost . ( We need this to properly figure out at the end of the month which kiosk has sent us which leads in an organized fashion and how much was the cost of the repair so that we can give a commission to the kiosk .) 4. And the user should be able to login to the application and send a email with all these reports to the appropriate kiosks . This email is sent out at the end of the month so we can provide proof of their work and then send them a check.
