Web Form to be sent as MS Word Attachment

Hi This should hopefully be an easy project We have had a webform built which all functions fine however we need the results to be submitted in MS Word format attachment that when printed fits exactly onto a A5 sized paper. The output file is nearly there though we require modification to the style and layout (see attached docs) This doc must also be able to be read / edited on an ipad.
