Form To Database – AJAX – PHP - SQL

Database Developer Needed ASAP! We need online forms copied and built into a database and installed on our hosting. The database will have an admin panel for data export and a separate login for users. Admin are should have a master login and a sub admin where admin users could not edit user/password. The database will reside on our servers and we will require that you do the install . We will supply the forms you need to copy and have videoed an example for you to view. The first database should be ready within 3 days of bid award. If you can build this type of database, have a fast turn-around time, and will be available for adding options and trouble shooting, we need you. We will also ask that you add options and trouble shoot our current database that is written in ajax and php. You must communication regularly using Skype during USA business hours and be fluent in English.
