Pearl script and sample html file that posts form data to an XML file

I would like to have a Pearl script written that will accept form data from an HTML page and save the passed form data to an XML file. The script would have to be able to save any form data that is passed, the XML tags should be the names of the form fields that are passed. I would also like a sample HTML file to illustrate how the script can be utilized. The XML file will need to be saved in a subdirectly under the folder where the HTML file will be hosted. The XML file naming convention should use the date and time of the form submission, down to the second to ensure unique file names for each submission.
