Simple Tweak To Friend Referral Web Tool

Hello, I need a tweak made to a 'friend referal" tool I'm using. The tool uses a database...visitors "opt-in" to get a free gift, and an account is automatically created for them. Then they are given their own friend referral link automatically... They send the link to their friends, and when 3 of their friends opt in using their link, the person gets another free gift. Here's the tool I'm using, so you can check it out: <[login to view URL]> Note: to do this project you'll need MY files for my installation because it's already been customized...but this is the core tool so you can see what it does. This tool has already been modified by one coder, but I can no longer reach him. Here's what I need: For the main index page where the vistor enters their email and password to opt-in and create their account... I need to be able to move that opt-in box around. I also need it to be more narrow... shorter fields. All the HTML that displays this page is retrieved from the mysql database. I need you to create a little "tag" I can put in the HTML that will be replaced with the opt-in box. Something like "##BOX HERE##". This will allow me to move the box around wherever I want in the HTML code. That's the first part...now this second part... I need 2 different opt-in pages, that do EXACTLY the same thing...except that I can change the look and feel of each... I need the ability to simply copy the page, give it a different filename like "[login to view URL]" and it will work just as the first one does, but will look different.
