97400 php web app install script

Hello, I require a php based install script to install a web app. Something that looks like this, but in php: [login to view URL] I would like to be able to get the FTP info from the user to wget the tar to the users webspace, then the install script will start on the users webspace to install the php app and database info. All I need is the basic structure of the form to get the info I need and the functions to wget the files on the server. Then the install script need to collect the users DB info and other info I will add myself later, to then proceed to untar the web app, and install the MySQL DB. I really need something quite simple, as I will fill in the blanks. Of course it should be in workable order when delivered. Cheers.
