Issues with PHP mail chimp integration on a purchased template.

I have purchased a template from envato market link below : [login to view URL] how ever after installing it and replacing the api key and list id. it give me an error in which you can see easily by trying to input an email and clicking 'notify me' There should be a simple adjustment to the php module to make it work. I can give you ftp access to all the files along with mail chimp access.
