I'm in need of an online hearing aid test for my wordpress based website, copied and based exactly like this one: [login to view URL] Think it more like a form with multiple step questions. Some of those questions will actually have a sound file which asks the user to click volume up and down buttons to find out what the minimum volume is the user can hear. This will be mixed in with multiple choice questions as shown on the example website. It will end with the user giving their name and contact details. The contact details would then need to be able to link up with an email autoresponder such as mailchimp or similar. I don't mind how this is done but it would seem to make sense as a plugin.
