Fillable webform / database

We need a webform/database where we can log the dives performed by our divers. Every form shall be given an automatic running number when the form is saved and sent (submit) The form shall be saved as a PDF and sent to a defined email address when submitted. In addition, the form should also be sent to personnel involved (chosen from a drop-down list) The webform/database shall be searchable so that we can easily go into the system and find all dives and bottom times of any of the divers in the system. The webform shall have dropdown lists where names are selected, type of diving gear, and so on. Since this is a dive journal, the form shall also include the time when the diver leaves the surface, bottom time etc. The bottom time shall be automatically calculated into minutes by the system, based on time spent underwater. It must be simple to use and not possible to damage the system for the users. Admin must be able to add, remove, edit personnel and personnel info that the system uses to make the forms. We already have a system but it is not 100% as intended. it has been made in ELEMENTOR and loaded into WP web page. But it will give you an idea of what we are looking for.
