Front-End Development: Add Two Pages to my WordPress Website

About me: I am a Health and Wellness coach and owner of a coaching business with my own website www.mindbodycreations.com. I have been maintaining/editing this website using Wordpress and Divi Builder. What I'm looking for: An experienced front-end web developer to add two new pages to my website. 1. New "Coaching Agreement" web page: a. Client enter name, email, phone number, select permissions, read agreement, and indicate agree to terms. b. After client agrees to terms, send auto-email confirmations to client and to me. c. In confirmation emails attach PDF copy of client entries, agreement, and confirmation. Note: I have agreement prepared (4-page PDF) and functionality of this web page defined in separate documents that I will provide after you express interest in this project and prior to your proposal so you are aware of the details. 2. "Client Assessment" web page: a. Client enter personal data via predefined selections or by entering text. b. After client completes entries, send auto-email confirmations to client and to me. c. In auto-emails include PDF copy of completed assessment to client and to me. d. If client doesn't complete assessment in one sitting, save information for client to return and complete later. e. Provide auto-emails to client and me for uncompleted assessments with URL links to return and continue with previous data entered. Note: I have assessment wording prepared (8-page PDF) and functionality of this web page defined in separate documents that I will provide after you express interest in this project and prior to your proposal so you are aware of the details. Note: These two new web pages and auto-emails need to set up so that in the future I can make my own edits to the text within Wordpress or some reasonable way. I have MS Office, but not full Adobe or other special software. 3. "Contact Us" web page a. Correct Subject line on email I receive. b. Is there a way to reduce spam without a recurring cost? 4. Currently Divi Builder is not letting me make my normal edits. Determine cause, communicate to me what is the problem, correct it so I can get back to making edits. Schedule expectation: I would like this completed as soon as possible and will select a provider based on schedule availability. To complete all the deliverables, the right developer will have experience in the following: - Web page development for WordPress. - Development of Forms with client entry. - How to send emails automatically based on client actions on web page.
