We have an ugly wordpress form that needs to be re-designed : See : [login to view URL] - content and questions are correct - design is not clear enough for customers to make the right selection among our 4 plans - design need to be user-friendly and in line with our current website colors This should be a 3-4 hours work for a wordpress specialist. Milestone one : Submit a redesigned form for approval (in line with website colors) Milestone two : Implement the approved form on our website and test functionalities Requirements : - No paying plugin - Please check our current theme to make sure you can implement your work without surprise at time of implementation. - All new submission should arrive directly to our email address (including attachement) - Need to be up and running within the next 4 days - No crazy price this is not a complex job and an example is provided - Please ask questions if something is unclear - Do not apply if you are not ready immediately - Please submit previous wordpress work
