php form

I have just completed a website for a client which is currently on the url ukdiamonds.info. What I need is a php form on the email santa page where they complete some fields, it will need to be the same as this [login to view URL] Once they have completed the form they need to be prompted to pay £1.99 via Paypal and then will get sent direct to a page with the letter from santa claus on it, and also the letter emailed to them if they have included an email. If you look at the above link [login to view URL] and actually try sending an email yourself you will see what I mean, but with theirs there is no charge and I will need a charge of £1.99 via paypal. The php involved will mean the letter they receive will be addressed to them and mention what they wanted for xmas and where they live etc. The wording can be more or less copied from the above site. It will also be necessary for the letter to show as soon as they have paid and also be emailed to them if they have provided an email address. This field should be optional. I need this job done urgently so please only quote if it can be completed very soon.
