I need a calculator tool with two sliders (see attachment) integrated into Qualtrics. It should calculate an output given two inputs by the sliders and a specific equation. I was able to create the desired calculator using [login to view URL]'s form widgets. I can even get the HTML code from it (mixed in with other code). However, what I need is to be integrate this calculator into a Qualtrics survey, either via JS or HTML.
