I am seeking to develop a survey utilizing the following: 1. An ideal response method- How true, How Important, and How Effective. 2. Weighing Scale (6 points) ranging from Extremely True, Very True, Somewhat True, Not Very True, Never True, I don't know. 3. There will be demographics (optional) 4. I will be able to break the responses out across many different graphs and also the responses based on answers. I have screenshots from a similar survey which I can share if needed. 5. I will need analytics tied into this, possibly leveraging Zoho Analytics and Zoho Surveys. I would need to see samples of previous work
