sign on page and work form

We need two things. First is a simple log on page with only a username and password window. Our art team can design the graphics. We need the Password to determine what page the user will be directed to. we are running php and my sql. we need an administration area to add users and pages latter. The second is a Form designed just like our work orders. this will include text field for Quantity etc. and Pulldown menu's for product sizes, color etc. and needs an area for artwork upload. This form will then be e-mailed to us, as well as an order confirmation to the client. we are an apparel decorator so product size is from youth XS to Adult XXXXXXL. artwork will upload to seven area's on work order. Front, Sleeve, left sleeve Right Leg, head-wear, Other etc. this is the beginning of many projects we have.
