Update a Medical Web Referral System

1) When the save button is clicked..... I would like some sort of message and redirect created when a person does not enter all required fields... Now it does not say anything or do anything... .2) Also I would like 'evaluation type' clickable to reveal the description Is In web eval type table.... 3) Also if evaluation type is the radio button associated with the cope Health and behavior assessment And If company name of the referral source is like wild card, (Look up company a table) wildcard THEN concatenate That Company name to the front of the string and save to the intake database as evaltype. For example if the company was Corvel and the type of assessment was health and behavior**** then you would save as CorVel health and behavior****
