Website design and graphics

I have an existing self improvement/personal growth blog and need the following work done: GRAPHICS: 1. Design header graphic to be used on both website and existing wordpress blog. The header graphic is to include or incorporate a yin-yang symbol muted or in shadow. 2. Design ebook cover in 3 sizes. 3. Animated gif using medium and small ebook covers WEBSITE: 1. 3-column website template design I can update/edit with Frontpage. I need 5 links in the nav bar, some will have dropdown sub-menus. 2. One-time-offer page that can only be seen only once by a visitor 3. Other: use includes for sidebars, footers, etc. I want to be able to make changes quickly with a minimum of work. Tell-a-friend feature on pages 4. Hover popup for email collection 5. Email collection page with question form (like askdatabase sites), a field with a drop-down list, also collecting data from a hidden field, date field, IP address, plus usual name and email fields. Must be compatible with Mailloop software. OTHER: I will need this completed by October 21. Meeting the deadline is very important. If you can't meet the deadline don't even bid. Please. P.S. This is the first time I'm hiring a freelancer so I'm not sure if the budget is reasonable. Bids over budget will be considered.
