Creating Upload Form

We are looking for someone to create an artist upload form for our website. The purpose of this form is to allow musicians and fans to upload their information to our website. We are currently using OSCommerce as our platform and need to make sure that the form works for both musicians and fans. We will also begin selling "downloads" for musicians so we need to have this for setup to upload mp3 files, as well as a picture and a video. We have included a link to view what we would like the final product to look like. We will also need a link created in our "admin" for all of the uploads to sit in until we "approve" or "decline" the uploads. Lastly, we will also need another category list to be created on our "Home" page for our "fans" information to appear in. We will provide the category list. ## Deliverables [login to view URL]
