medical translation needed from German to English

I have a two page (single spaced) medical report written in German that i need translated into English. Would prefer either a professional medical translator or a doctor for this. Preferably in Germany but as long as you are compitant, location isnt a deal-breaker. Please apply with either a resume or examples of your past medical translation work. Also please include the words "jumbo jet" in all bids and pm's to ensure that you have fully read this project. All bids without the words will be ignored.
